You need to bring proof of current address, identification, and income. You can use the check list on this website to help you.
What do I expect at my first visit?

What happens at your first certification visit will depend on whether you are applying for yourself or for a child and how much information you bring with you. In general though you will first talk to a clerk who will ask questions about your address, phone number, income and other family information. The clerk will then determine whether you meet the income and residency requirements. You will be asked to complete a form that gives the nutritionist information about what you or your children eat. If you did not bring medical information from your doctor we will then take some blood for a hemoglobin test and obtain a weight and height. Your next stop will be with a nutritionist who will assess the weight, height, hemoglobin, diet and medical history to determine if you (or your child) have a nutrition risk that would make you eligible for the program. The nutritionist will work with you to decide what you could do to make improvements in your family's nutrition and to answer your questions. You may be asked to help choose the food package that you need and can use. When you are finished you will then return to the clerk who will print your checks and explain how to use them. You will then be given an appointment to come back and pick up more checks usually in three months. You should plan on a minimum of 30 minutes for certification visits but pick up visits should only be 5-10 minutes.
How often do I need to be recertified?

Children need to be recertified every six months. Pregnant women need to be recertified 6 weeks after delivery. Infants are certified to their first birthday but should have a weight height and hemoglobin check shortly after age 6 months. Breastfeeding women can be eligible until the baby is one year old but will need to be recertified in six months.

Since most of the WIC sites in this area are not open full time, the best way to contact WIC is to call the main office in Gainesville. They can answer most questions and make or change appointments.
